Appointments
The first step toward a beautiful, healthy smile is to schedule an appointment. If you are a patient of record, please call our office to schedule an appointment.
If you are a new patient, thank you for your interest in our clinic! To provide you the best care, please follow the checklist below.
New Patient Checklist:
1) Dental Insurance:
If you would like to make sure that our doctors are in your insurance network, please call the number on the back of your insurance card.
2) New Patient Forms:
Please call our office at 507-744-2359 to have our patient forms sent to you either by text message, or email. If you do not have the resources to fill these forms out by text or email, please let us know and we will be happy to print them out for you, or mail them to you.
3) What to bring with you to your appointment:
-Please bring with you a list of medications that you are currently taking
-If you have been told by your medical doctor that you need to take an antibiotic before your dental visits, please notify us and take as prescribed by your physician.
-Please bring a picture I.D.
-Please bring your dental insurance card. If you have an electronic version, please email a picture of the front and back of the card to info@lonsdalefamilydental.com.
